What care covers

What does concierge primary care include at PrimaryMD?

It includes the work of an ongoing primary-care practice, from a new symptom or medication question to chronic-condition management and specialist coordination. Each decision returns to one physician-led plan instead of starting over at every visit.

Acute concerns and same-day appointments when needed
Ongoing management of chronic conditions and risk factors
Medication review, prescribing, renewals, and reconciliation when another clinician changes the list
Preventive screening and immunization planning based on age, history, risk, and prior care
Review of relevant labs, imaging, wearable trends, and outside records
Specialist referrals, records coordination, and follow-up after recommendations come back

Telehealth is available when clinically appropriate. Urgent needs after hours or while traveling are supported through Sollis Health under applicable membership terms. PrimaryMD does not replace health insurance. Hospitalizations, procedures, specialist services, and care outside the membership remain subject to insurance or separate charges.

How care stays connected

What happens when something changes?

A symptom, medication change, or specialist recommendation enters one shared thread. Your physician owns the clinical decision; the care coordinator moves records, referrals, and scheduling; the outcome returns to the same plan.

Nothing important lives in a separate thread.

Why continuity matters

What does a long-term primary-care relationship change?

Continuity lets your physician compare today’s question with what came before: prior symptoms, medications, labs, imaging, specialist input, and what did or did not work. That context does not guarantee a simple answer, but it supports a clearer decision and a defined next step.

Care should not reset every time something changes.

The PrimaryMD care team reviews a member’s records together.

Your physician does not replace every specialist. Your physician holds the full context, coordinates the handoffs, and keeps the plan current.

The PrimaryMD care model

PrimaryMD sells no supplements, marks up no tests, and owns no imaging centers.

Recommendations are based on what can clarify a decision or change care. Outside services may carry separate costs, and PrimaryMD will tell members when they do.

Common questions

Concierge primary care at PrimaryMD

Is PrimaryMD my actual primary-care practice?

Yes. PrimaryMD manages everyday primary care, acute needs, chronic conditions, medications, prevention, specialist coordination, and complex decisions.

Who is concierge primary care for, and when might it not be the right fit?

It is for adults who want PrimaryMD to serve as their ongoing primary-care practice and value continuity, access, coordination, and follow-through. It may not be the right fit if you only want occasional standalone visits or testing. Membership does not replace health insurance, specialists, hospital care, or emergency services, and it does not guarantee that every concern can be handled remotely or immediately.

How are chronic conditions and medications managed?

Your physician reviews symptoms, relevant trends, treatment response, side effects, and the rest of your medication list before deciding whether to continue, adjust, monitor, or refer. Follow-up timing depends on the condition and decision.

Does PrimaryMD replace my specialists?

No. Specialists remain part of your care and are generally billed through insurance. PrimaryMD coordinates referrals and records, reviews outside recommendations in context, reconciles medications when needed, and returns the outcome to your plan.

What happens after hours or while I am traveling?

For urgent needs, Sollis Health provides partner access under applicable membership terms. PrimaryMD connects the episode back to your ongoing plan and follow-through. Confirm available markets and applicable terms with the membership team.

What should I do in a medical emergency?

Call 911 or go to the nearest emergency department. Do not use the website, secure messaging, or a waitlist form for an emergency. Once immediate care is underway, PrimaryMD can help connect the episode back to your ongoing care.

This page describes what PrimaryMD’s membership includes and is intended for general information. It is not medical advice and does not create a physician-patient relationship. Decisions about your own care should be made with your physician.
