The connected model

Five parts of care, one plan.

Everyday care, prevention, diagnostics, performance and coordination all feed the same place: your physician’s judgment about what matters for you. That judgment becomes one plan, and the plan is revisited as your health, your goals and the evidence change.

More data does not automatically produce better decisions. Closing the gap between information and action is your physician’s job, and it is the part of the model everything else is built around.

Dr. Sukhjit Takhar

“Right now, much of longevity medicine should be based on having a really good doctor to help guide your health.”

Sukhjit “Sarge” Takhar, MD, MS · Co-Founder and Chief Medical Officer · in Forbes, January 2026
Longevity medicine

Is this longevity medicine?

Yes, though the term has become slippery. Longevity medicine adds a layer to conventional care: longer visits focused on cardiometabolic fitness, sleep and diet, rather than treating illness only once it has arrived.

It should not be a separate specialty you visit alongside your actual doctor, paying twice for two people who never speak to each other. PrimaryMD was built on the opposite premise. The physician managing your blood pressure and your chest cold is the same one reading your genome and your VO₂ max.

There is no separate longevity program to buy here. It is part of membership because the continuity is what makes it work at all.

01
Everyday & ongoing primary care

What does everyday primary care include?

Everyday primary care at PrimaryMD covers acute concerns, chronic-condition management, medication management and quarterly physician visits, with same-day appointments available when needed, in person or by telehealth. It does not stop at the annual physical: your physician manages new problems as they arise and long-term conditions over years, with your history already in view rather than reconstructed at every visit.

New concerns are considered alongside your history, medications, recent results and ongoing plan, then given a next step rather than only a prescription.

Acute concerns and same-day appointments when needed
Chronic-condition management over years, not visits
Medication management and reconciliation
Quarterly physician visits, plus telehealth

Panel sizes are capped so your physician has time to know your history rather than re-read it.

Prevention & diagnostics

What prevention and diagnostics does membership include?

Membership includes quarterly blood panels covering 100 biomarkers, twice-yearly DEXA body-composition and bone-density scans, VO₂ max and resting-metabolic-rate testing, and clinical whole-genome analysis. Prevention starts with a meaningful baseline rather than a checklist, and your physician decides what to act on, what to monitor, and what to leave alone.

Quarterly comprehensive blood panels, with mobile phlebotomy
Twice-yearly DEXA body-composition and bone-density scans
VO₂ max and resting-metabolic-rate testing
Clinical whole-genome analysis

Measure broadly. Interpret carefully. Act selectively. Follow continuously.

Dr. Fred Watson

“The most powerful cardiology isn’t done in a cath lab. It’s done years earlier, with a plan built around you. My goal is to help you understand your heart, act early, and feel confident about the decades ahead.”

Fred Watson, MD, FACC · Preventive Cardiologist

How we decide what is worth testing

Some tests clarify a decision. Others generate findings that lead nowhere, or create anxiety without changing care. Imaging and expanded genomic testing are available when they are clinically appropriate, and your physician will tell you when a test is unlikely to help.

PrimaryMD sells no supplements, marks up no tests, and owns no imaging centers.

When our physicians recommend a test, nothing about that recommendation is influenced by what it earns us.

Fitness & metabolic health

How does testing become an actual plan?

Your exercise physiologist turns DEXA, VO₂ max and metabolic-rate results into a specific, progressive plan, integrated with wearable and continuous glucose monitor data and aligned with your physician's priorities. Testing only matters if it changes what you do.

Heather Milton, our Lead Exercise Physiologist, is a registered clinical exercise physiologist and Certified Strength and Conditioning Specialist with more than eighteen years of experience across clinical exercise physiology, sports performance and research.

Specialist & complex-care coordination

Who coordinates specialists and complex care?

Your physician coordinates specialist care, reviewing what each specialist finds, reconciling medications afterward, and following up on recommendations. A specialist visit produces a report, and someone still has to decide what it means for everything else, so a cardiology note and a sleep study do not sit in two separate files with no one connecting them.

Your dedicated care coordinator manages records, referrals and scheduling, so a recommendation does not stall between people. Outside specialist services are not included in membership and may be billed separately or through your insurance.

Reading a specialist’s report critically is a skill

Our Chief Science Officer, Dr. Michael Billington, is board-certified in Emergency Medicine and Critical Care, serves as Deputy Editor at NEJM Journal Watch, and directs the Emergency Medicine and Critical Care Fellowship at Brigham and Women’s Hospital. Evaluating clinical evidence is his day job.

Dr. Michael Billington

“My passion lies in delivering patient-centered critical care through innovative education, meaningful communication, and advancing clinical excellence.”

Michael Billington, MD · Chief Science Officer, PrimaryMD

Dr. Billington has also written for the medical drama The Resident, an experience he credits with shaping how he communicates clinical information to patients.

Service FAQ

Common questions about what we cover

Is PrimaryMD my primary-care practice?

Yes. PrimaryMD manages everyday primary care, acute needs, chronic conditions, medications, prevention, specialist coordination and complex decisions.

Does PrimaryMD replace my specialists?

No. Specialists remain part of your care. Outside specialist services are not included in membership and may be billed separately or through your insurance. Your physician reviews what they find, reconciles medications, and folds the outcome back into your plan.

How are diagnostics selected?

Membership includes a defined diagnostic program. Beyond that, your physician recommends additional testing only when it can clarify a decision, and will tell you when a test is unlikely to help.

Do you profit from the tests you recommend?

No. PrimaryMD sells no supplements, marks up no tests, and owns no imaging centers. When an outside service carries an additional cost, your physician’s recommendation is based on clinical value, not revenue to PrimaryMD.

How do results become a plan?

Every finding is given a next step: act, monitor, repeat, refer, or leave alone. Those decisions become one prioritized plan, revisited as your health, goals and the evidence change.

What happens at night, on weekends, or while I am traveling?

Membership includes 24/7 access to your care team. For urgent needs, Sollis Health provides partner access under applicable membership terms, and whatever happens is connected back to your physician and your ongoing plan.

Where is PrimaryMD available?

PrimaryMD serves New York City, New Jersey and the San Francisco Bay Area, with clinics in Hudson Yards, SoHo and San Mateo, and approved partner access in several additional markets.

Membership is $1,500 per month, billed annually, and is intentionally limited to protect the time and continuity the model requires. PrimaryMD does not replace health insurance. The first conversation is designed to determine mutual fit before either side makes a commitment.
