For Monta Vista and the rest of Cupertino, PrimaryMD offers a physician-led team of four that manages your health as one connected picture. The work starts with your history, as past records, medications, and family history are gathered in the first two weeks, followed by a two-hour intake, a baseline, and one plan. Your exercise physiologist meets you monthly, and your physician reviews your progress each quarter. Physician visits are held at PrimaryMD San Mateo or by video, and for lab work, urgent needs, or after-hours care you can use the Palo Alto Center, part of our care network.
Foothill Expressway and I-280 both run from the Cupertino foothills toward Palo Alto, where our care network's Palo Alto Center occupies Suite 102 at 555 Middlefield Road. Its doors open at 7 a.m. and close at 11 p.m. Monday to Friday; on weekends, hours are 9 a.m. to 5 p.m. Physician visits happen up the Peninsula at PrimaryMD San Mateo, 101 South San Mateo Drive, on weekdays between the hours of 8:30 a.m. and 5:30 p.m. In the city, our San Francisco 24/7 Center on Van Ness Avenue is open 24/7.
Traditional primary care panels often run into the thousands. Each PrimaryMD care team is capped at 150 members, so your team has time to know you.

Return to your plan through recurring clinical review. Your team brings new information and changing needs into the conversation.

Yes. The Palo Alto Center keeps its doors open until 11 p.m. on weeknights, and the San Francisco 24/7 Center stays open around the clock, with your PrimaryMD team following up on the visit. For an emergency, call 911.
Your team stays in touch by video, phone, or text, reviews wearable and glucose-monitor data for trends, and meets you monthly for an exercise physiology session.
