If you live on Ocean Avenue, PrimaryMD gives you a physician-led team that works with you by video, phone, text, and email, and our Santa Monica Center is close by for exams, lab work, and urgent needs. The team of four is led by your physician, with a care coordinator, an exercise physiologist, and a medical assistant. Membership opens with your records, a two-hour intake, and six weeks of baseline work, then a plan that sorts findings into what to act on now, what to keep watching, and what can be left alone. Quarterly reviews keep it current.
Wilshire Boulevard ends at Ocean Avenue, and our Santa Monica Center is farther up the same boulevard, at 2811 Wilshire Boulevard, Suite 850. It is open from 7 a.m. to 11 p.m. Monday to Friday, with weekend hours of 8 a.m. to 8 p.m. The rest of your baseline comes to you or close by: blood draws and a sleep study at home, a glucose monitor you wear, and DEXA and VO2 max at a partner center your care team books. We send your history ahead each time.
Traditional primary care panels often run into the thousands. Each PrimaryMD care team is capped at 150 members, so your team has time to know you.

Not on Ocean Avenue itself. Our nearest care network location is the Santa Monica Center at 2811 Wilshire Boulevard, and we also have centers in Beverly Hills and Manhattan Beach.
Mostly at home, with the rest at partner centers near you. A mobile phlebotomist handles blood work, the sleep study and glucose monitor are done at home, and your team books DEXA, VO2 max, and imaging your physician recommends.
At the Beverly Hills 24/7 Center, which never closes, at 155 North San Vicente Boulevard. On weekday evenings the Santa Monica Center is another option until 11 p.m., and your PrimaryMD team follows up after either visit.
It is $1,500 per month, billed annually ($18,000 per year), which is what membership costs in every other market too. Membership does not replace health insurance, which you keep for hospital care, specialists, procedures, and medications.
