Blackhawk members of PrimaryMD work with a physician-led team of four, and much of that work happens at home. Blood panels are drawn at your kitchen table, the seven-night sleep study runs in your own bed, and wearable and glucose-monitor data are reviewed by the team for trends. A two-hour intake starts things off, and a quarterly review keeps the plan current. You see your physician in person at PrimaryMD San Mateo or by video, and lab work, urgent needs, and after-hours care are handled at centers in our care network.
From Blackhawk, Crow Canyon Road leads to I-680 in San Ramon, and I-680 south to I-580 brings you to the San Mateo Bridge (SR-92) and PrimaryMD San Mateo at 101 South San Mateo Drive, where physician visits run weekdays from 8:30 a.m. to 5:30 p.m. Around the clock, members can also use the San Francisco 24/7 Center in our care network, located at 1005 Van Ness Avenue. The Palo Alto Center (555 Middlefield Road, Suite 102) keeps weekday hours of 7 a.m. to 11 p.m. and weekend hours of 9 a.m. to 5 p.m.
Traditional primary care panels often run into the thousands. Each PrimaryMD care team is capped at 150 members, so your team has time to know you.

Yes, though both late options are across the bay: the San Francisco 24/7 Center is open at any hour, and the Palo Alto Center takes visits until 11 p.m. Monday to Friday. Your care team hears about the visit and follows up. For an emergency, call 911.
No. Routine draws happen at your home or office with a mobile phlebotomist, and your physician reviews the results with you.
