PrimaryMD's physician-led team for Lafayette members has four people: a lead physician who knows your history, an exercise physiologist, a care coordinator who handles the logistics, and a medical assistant. Your first weeks bring a two-hour intake and a six-week baseline, which become one plan you revisit each quarter. Blood work is drawn at home, wearable data is reviewed for trends, and many conversations happen by video. In-person physician visits are at PrimaryMD San Mateo, and urgent needs, lab work, and after-hours care go through our care network.
Lafayette BART on the Yellow Line and SR-24 through the Caldecott Tunnel are the main routes west. Across the bay, the San Francisco 24/7 Center welcomes members around the clock at 1005 Van Ness Avenue; it is part of our care network, and Civic Center is the nearest BART stop. Physician visits take place at PrimaryMD San Mateo, 101 South San Mateo Drive, where weekday hours start at 8:30 a.m. and run to 5:30 p.m.
Traditional primary care panels often run into the thousands. Each PrimaryMD care team is capped at 150 members, so your team has time to know you.
