PrimaryMD works on the Upper West Side through a physician-led team of four who handle your care as one relationship, with physician visits at PrimaryMD Hudson Yards or by video. You start with a two-hour intake and a baseline: home blood draws, whole-genome sequencing, a seven-night sleep study in your own bed, and fitness testing with a clinical exercise physiologist. Each quarter, your physician reviews one focus with you, and in between your team follows the trends in your wearable data. Across the park, our care network's Fifth Avenue Center handles lab work and urgent needs every day.
The nearest in-person stop is across Central Park, where the Fifth Avenue Center at 945 Fifth Avenue keeps daily hours of 7 a.m. to 7 p.m.; a crosstown bus gets you there. Further east, at 170 East 77th Street, the Upper East Side 24/7 Center never closes. For physician visits, ride the C down Central Park West to Penn Station and walk over to PrimaryMD Hudson Yards at 503 West 35th Street, which opens at 8:30 a.m. and closes at 5:30 p.m., Monday to Friday.
Traditional primary care panels often run into the thousands. Each PrimaryMD care team is capped at 150 members, so your team has time to know you.

The Fifth Avenue Center, part of our care network, is closest: it sits across Central Park at 945 Fifth Avenue and is open every day from 7 a.m. to 7 p.m. You see your physician at PrimaryMD Hudson Yards, 503 West 35th Street, or by video.
Can I be seen after hours on the Upper West Side?
Yes. Across the park, the Upper East Side 24/7 Center never closes, and our Hudson Yards Center is open daily until 8 p.m. Your care team follows up after either visit.
How do I get across town from the Upper West Side?
A crosstown bus through Central Park brings you to the East Side, near the Fifth Avenue Center and the Upper East Side 24/7 Center. For physician visits, take the C to Penn Station, or ride the 1 train to Times Square and switch to the 7 for PrimaryMD Hudson Yards.
