In Carroll Gardens, PrimaryMD works as a long-term relationship with a physician-led team of four who come to know you well: your history, your goals, and what a normal week asks of you. It begins with a two-hour intake and a baseline that includes blood work, whole-genome sequencing, a seven-night sleep study at home, and physiology testing with an exercise physiologist. In-person visits with your physician are at PrimaryMD SoHo, our flagship, and the rest can happen on video; the Tribeca Center, part of our care network, is where Carroll Gardens members go for lab draws, urgent visits, and evening care.
The F and G stop at Carroll Street. Take the F to Jay Street-MetroTech and switch to the A or C for Chambers Street and the Tribeca Center, on the 5th Floor of 255 Greenwich Street and open 7:30 a.m. to 7:30 p.m. daily. The C also continues to Spring Street for PrimaryMD SoHo at 430 West Broadway, where physician visits run 8:30 a.m. to 5:30 p.m. on weekdays. Staying on the F to 23rd Street brings you close to the Gramercy Center at 11 East 22nd Street, which is open weekdays, 9 a.m. to 5 p.m.
Traditional primary care panels often run into the thousands. Each PrimaryMD care team is capped at 150 members, so your team has time to know you.

Weeks one and two gather your story, weeks two to four set the baseline, and by week six you have one plan with a next step for every finding: act, monitor, repeat, refer, or leave alone. Yes. Members can use the Tribeca Center until 7:30 p.m. daily, and our Upper East Side 24/7 Center on East 77th Street stays open overnight; your team follows up afterward and updates your plan. For an emergency, call 911.
