PrimaryMD gives Brooklyn Heights residents a physician-led team of four who get to know their history, their goals, and how their days actually run. Membership opens with six weeks of groundwork: records and specialist notes gathered, a two-hour intake with your physician, and a plan in which every finding has a next step. After that, your team stays with you through quarterly reviews and everyday questions. You see your physician at PrimaryMD SoHo, our flagship, or by video. When you need lab draws, urgent care, or an after-hours visit, the Tribeca Center, part of our care network, is close by.
From Clark Street or Borough Hall, the 2 and 3 run to Chambers Street, a short walk from the Tribeca Center at 255 Greenwich Street, 5th Floor, open from 7:30 a.m. until 7:30 p.m. seven days a week. For physician visits, take the R from Court Street to Prince Street or the C from High Street to Spring Street; either leaves you a short walk from PrimaryMD SoHo at 430 West Broadway, open 8:30 a.m. to 5:30 p.m. every weekday.
Traditional primary care panels often run into the thousands. Each PrimaryMD care team is capped at 150 members, so your team has time to know you.

PrimaryMD brings primary care, prevention, and ongoing follow-through into one physician-led relationship. Your care team gets to know your history, priorities, and daily life, then works with you on a plan that can evolve over time.
Begin with your health history, questions, and goals. Your physician uses that context to guide a meaningful baseline assessment.
Work with a dedicated care team on shared priorities and practical next steps, with support for everyday primary care.
Return to your plan through recurring clinical review. Your team brings new information and changing needs into the conversation.

The Tribeca Center, part of our care network, is closest: 255 Greenwich Street, 5th Floor, in Lower Manhattan, with hours seven days a week from 7:30 a.m. to 7:30 p.m. You see your physician on weekdays at PrimaryMD SoHo, 430 West Broadway.
Yes. The Tribeca Center, a few stops away on the 2 or 3 train, sees members every evening until 7:30 p.m.; after that, the Upper East Side 24/7 Center is open around the clock, and your PrimaryMD team hears about either visit. For an emergency, call 911.
Take the 2 or 3 from Clark Street to Chambers Street for the Tribeca Center. For physician visits at PrimaryMD SoHo, ride the R from Court Street to Prince Street or the C from High Street to Spring Street.
