CARE IN YOUR AREA
Explore the area
Joining PrimaryMD from Forest Hills Gardens gives you a physician-led team of four that builds a full picture of your health and keeps it current. Much of the baseline happens at home: blood work by a mobile phlebotomist and a seven-night sleep study, alongside a two-hour intake and physiology testing with your exercise physiologist. Physician visits are at PrimaryMD Hudson Yards or by video. Upstairs, the Hudson Yards Center in our care network is open every day for lab work, urgent needs, and after-hours care.
Station Square is the starting point. From the Forest Hills LIRR station, trains run to Penn Station, a short walk from 503 West 35th Street. That building holds PrimaryMD Hudson Yards, which sees members from 8:30 a.m. to 5:30 p.m., Monday to Friday, and the Hudson Yards Center, open daily, 8 a.m. to 8 p.m. Trains to Grand Central Madison connect with the 6 train, which stops at 77th Street by the Upper East Side 24/7 Center (170 East 77th Street, open 24/7). The E and F on Queens Boulevard are another option.
Traditional primary care panels often run into the thousands. Each PrimaryMD care team is capped at 150 members, so your team has time to know you.

Discuss care in your areaConcierge care improves time and access, and PrimaryMD uses both to bring more of your care home. A mobile phlebotomist handles blood draws, the sleep study runs for seven nights where you actually sleep, and many follow-ups happen by video or text, while your team coordinates specialists and records in the background. Nothing we recommend is tied to a sale, since we sell no supplements and do not mark up tests.

HOW MEMBERSHIP WORKS
PrimaryMD brings primary care, prevention, and ongoing follow-through into one physician-led relationship. Your care team gets to know your history, priorities, and daily life, then works with you on a plan that can evolve over time.
Begin with your health history, questions, and goals. Your physician uses that context to guide a meaningful baseline assessment.
Work with a dedicated care team on shared priorities and practical next steps, with support for everyday primary care.
Return to your plan through recurring clinical review. Your team brings new information and changing needs into the conversation.
MEMBERSHIP AT A GLANCE
Membership is not health insurance. Insurance remains important for hospital care, specialists, procedures, medications, and services outside the membership.
