For Great Neck residents, PrimaryMD means one physician-led team of four that knows your history and stays with you from year to year. You begin with a two-hour intake and a baseline: blood work drawn at home or at the office, whole-genome sequencing, a seven-night sleep study at home, and DEXA, VO2 max, resting metabolic rate, and grip strength with a clinical exercise physiologist. Every quarter, your physician reviews one focus with you. In person, you see your physician at PrimaryMD Hudson Yards or PrimaryMD SoHo, while our care network's Manhattan centers take care of lab work and urgent needs.
The LIRR Port Washington Branch runs from Great Neck to Penn Station and Grand Central Madison. From Penn Station, a short walk west brings you to 503 West 35th Street, where PrimaryMD Hudson Yards (Monday to Friday, 8:30 a.m. to 5:30 p.m.) shares a building with the Hudson Yards Center, open every day from 8 a.m. to 8 p.m. If your train ends at Grand Central Madison, the 6 train is close by, running up to 77th Street and the Upper East Side 24/7 Center, which is always open.
Traditional primary care panels often run into the thousands. Each PrimaryMD care team is capped at 150 members, so your team has time to know you.

PrimaryMD Hudson Yards, near Penn Station at 503 West 35th Street, is the nearest place to see your physician, Monday to Friday. The Hudson Yards Center, part of our care network, is in the same building and open daily from 8 a.m. to 8 p.m. for lab work and urgent needs.
Yes, in Manhattan, where the Upper East Side 24/7 Center (170 East 77th Street) never closes and the Hudson Yards Center near Penn Station is open daily until 8 p.m. For an emergency, call 911. Either way, your PrimaryMD team picks up the follow-up.
Same-day access is part of membership: you can reach your physician by video or text, or be seen in person at a PrimaryMD clinic, so a question does not have to wait for a trip on the LIRR.
