PrimaryMD care in Rye starts with a physician-led team of four who get to know your history, your family, and what you want from your health. Membership opens with six weeks of groundwork: your records collected, two hours with your physician at intake, and a baseline covering blood work at home, whole-genome sequencing, and a seven-night sleep study in your own bed. You come away with one prioritized plan, revisited each quarter. Your physician sees you at PrimaryMD Hudson Yards or PrimaryMD SoHo, or by video, and lab work, urgent needs, and after-hours care go through our care network in Manhattan.
New Haven Line trains run from Rye to Grand Central, and the 7 train heads west from there to PrimaryMD Hudson Yards, on the 2nd floor of 503 West 35th Street and open Monday to Friday, from 8:30 a.m. until 5:30 p.m. Upstairs, the Hudson Yards Center, part of our care network, keeps longer hours, from 8 a.m. to 8 p.m. every day. The 6 train from Grand Central runs up to 77th Street and the Upper East Side 24/7 Center at 170 East 77th Street, which never closes.
Traditional primary care panels often run into the thousands. Each PrimaryMD care team is capped at 150 members, so your team has time to know you.

Yes, in Manhattan: our care network's Upper East Side 24/7 Center, off the 6 train at 77th Street, is open around the clock, and the Fifth Avenue Center nearby keeps daily hours until 7 p.m. For an emergency, call 911. Anything that happens there goes back to your PrimaryMD team for follow-up.
Take the Metro-North New Haven Line to Grand Central, then the 7 train to Hudson Yards or the 6 train to 77th Street. Many check-ins happen by video, phone, or text, so not every question needs the trip.
